If applying for a scholarship – Due prior to April 3:
Course Registration (Fellows in Training)                            $300.00 
Fellow/student scholarships available with a reduced registration fee of $300. Application on the Scholarships tab – must be submitted by Friday, April 3, 2020 to qualify.

After Thursday, May 7, 2020:

Mayo Clinic Department of Cardiovascular Medicine implemented an online conference registration system in January 2012.  If this is your first time using the online system, you will need to create a profile.  The attendee’s email address is required upon registration.  A letter of confirmation and receipt will be forwarded to this email address upon receipt of submitted registration and payment.  Present this letter when checking in at the meeting registration desk.

The registration fee includes tuition, electronic course syllabus, continental breakfasts, break refreshments, and lunch.

Although it is not Department of Cardiovascular Medicine policy to limit the number of registrants for a course, conference room facilities may necessitate closing of enrollment; therefore, early registration is advised.  Enrollment for this program is limited. 

CANCELLATION POLICY
If you cancel your participation in this course, your registration fee, less a $75 administration fee, will be refunded when written notification is received by Mayo Clinic CV CME before May 22, 2020 (email: cvcme@mayo.edu). No refunds will be made on or after May 22, 2020. Canceled registrations are non-transferable.

NO SHOW POLICY
This course is intended to be a live learning experience. If you do not attend this course on-site as a live learner, you will not be allowed access to the materials/lectures during or after the course. No refunds will be given for no-show registrants.

In the unlikely event Mayo Clinic CV CME must cancel or postpone this course, Mayo Clinic CV CME will refund the registration fee, but is not responsible for any related costs, charges, or expenses to participants, including fees assessed by airline/travel/lodging agencies.
