Live in-Person Registration
Registration fee includes tuition, electronic course syllabus, continental breakfasts, break refreshments,  and access to course presentations with video recordings after completion of the course for one year.

Livestream Registration
Registration fee includes tuition, live virutal program viewing, electronic course syllabus, submission of online questions, and access to course presentations with voice-over recordings after completion of the course for one year.  Program will be livestream in mountain time zone.

- Viewing during livestream times recommended for optimal learning
- CME credit may be claimed for up to one year post course
- ABIM MOC points must be claimed within 30 days post course

Although it is not Mayo Clinic Cardiovascular CME (CV CME) policy to limit the number of registrants for a course, conference room facilities may necessitate closing of enrollment; therefore, early registration is advised. Enrollment for this program is limited.

CANCELLATION POLICY
If you cancel your participation in this course, your registration fee, less a $75 administrative fee, will be refunded when written notification is received by Mayo Clinic CV CME before January 26, 2022 (email: cvcme@mayo.edu). No refunds will be made on or after January 26, 2022. Canceled registrations are non-transferable.

Mayo Clinic CV CME reserves the right to cancel or postpone any course due to unforeseen circumstances. In the unlikely event Mayo Clinic CV CME must cancel or postpone this course, Mayo Clinic CV CME will refund the registration fee, but is not responsible for any related costs, charges, or expenses to participants, including fees assessed by airline/travel/lodging agencies.
