Overview

This module will provide an overview of the history and development of a disease management program administered by Registered Nurses (RNs). The logistics of this type of care delivery model are supported by design aspects of other successful models as well as evidence-based studies that illustrate the effectiveness of RN managed care. The overall goal of this presentation is to provide the learner with basic elements of information that can be used toward the replication of a similar program.
