In this tutorial, we build on the skills discussed in the introduction to advance care planning, the presenter will discuss the importance of discussing goals of care and prognosis for those patients with advanced circulatory failure as well as the barriers and opportunities available to move these discussions earlier in the course of the patients illness. In addition, the presenter will discuss a sample framework for approaching advance care planning that can be utilized in a number of clinical settings.

  • Review rationale for discussions about goals of care with heart failure patients with poor prognoses
  • Identify opportunities for, and benefits of conversations about end-of-life (EOL) issues in heart failure practice
  • Describe barriers to discussion of EOL issues in heart failure
  • Propose a framework for discussing values and goals for EOL care
